Developing our Social Change Model

We have pioneered our Social Change Model (SCM) – a game-changer in integrated care. From medical to dental to behavioral health services, plus education and social services, we have got you covered, all under one roof where you feel at home.

The success of our model was further demonstrated by the final report of an evaluation that we solicited from the Urban Institute and analysis from the National Institutes of Health (NIH) which explains how this integrated approach has been shown to make people healthier.
